Samples Ordering Instructions Completely fill out the samples request form to the left of these instructions -- be sure to use the scroll bar to access the entire form. After verifying the information that you supplied, Asteame Medical Devices will send up to 2 samples to your doctor/surgeon at his/her office. Provided that we receive your request prior to your surgery, we will attempt (but do not guarantee) to ship the sample(s) to your surgeon prior to your first postoperative appointment. Asteame Medical Devices, Inc. reserves the right to decline any request at its discretion. |
